Question:
How can I ensure that the IMEI number I enter in the DevCertRequest tool is in the correct format?

Answer:
Use the key combination *#06# on the phone for which you want a Developer Certificate, and enter exactly the same IMEI number in DevCertRequest as displayed on the screen. This number may include hyphens or other non alphanumeric characters which should be entered as well. See Developer Certificates for more details.

Note - Do not enter any other IMEI format - like reading from the back of the phone, battery etc.
